Apply at your local DMV/DVS in your state. Just like you would apply for a vehicle title duplicate.

Check with the agency that regulates the MH industry in your State or juristiction. They should have a title division. If the home is regulated by the department of transportation or motor vehicles you will need to contact them about a duplicate.
